Boom Lift Operator NEEDED ASAP

1. Scope
Qualified boom lift operator to support facility
maintenance activities at the Honolulu VA Center for Aging in Honolulu, Hawaii.
The operator will be responsible solely for the safe operation and positioning of the boom lift, insuring the safe transport of maintenance personnel to and from elevated work locations.
- The operator is not responsible for performing the exhaust fan maintenance work.
Maintenance work will be performed by other contractor personnel.

2. Equipment
Equipment: 80' Articulating Boom Lift with Jib
Make/Model: Genie Z-80/60
Serial Number: Z8016H-6190

3. Operator Responsibilities
The operator shall:
� Perform required pre-use/daily equipment inspections prior to operation.
� Safely operate the Genie Z-80/60 80' articulating boom lift with jib.
� Position contractor personnel at exterior exhaust fan locations as directed by the GC site representative.
� Reposition the boom lift between work areas as required.
� Maintain safe positioning of the equipment while personnel perform work from the
platform.
� Follow manufacturer operating requirements and applicable OSHA aerial lift safety
requirements.
� Coordinate equipment movements with the site representative and
personnel working from the platform.
� Immediately report equipment defects, unsafe conditions, or operational concerns to the site representative.

The operator is not responsible for performing the exhaust fan maintenance work.

Day Shift: 8:00 AM � 4:00 PM
Estimated Duration: 8 hours

Night Shift: Beginning at 7:30 PM
Estimated Duration: 8 hours
Total Estimated Operator Hours: 24 hours
